Root Cause Analysis: Missing Item Modifiers on Orders
Impact Window: 00:10 IST – 01:00 IST (13:40 CDT – 14:30 CDT, prior day)
Impact: During this window, orders were ingested without their associated item modifiers.
What Happened
An issue at the database layer caused our modifier ingestion flow to silently fail. Orders continued to be ingested, but the modifier data associated with them was not written through correctly.
Why It Wasn’t Caught Sooner
We had monitoring in place specifically designed to pre-emptively detect this class of error before it could impact production orders. In this instance, however, this particular failure mode was not covered correctly by the existing checks, which delayed detection.
Resolution
Once identified, the team root-caused the issue and applied a fix, which fully restored normal ingestion. The fix was monitored to confirm stability before the incident was closed.
Preventative Actions
• Updated our monitoring scripts to explicitly cover this failure case.
• Completed a thorough review of our broader database monitoring to confirm there are no other coverage gaps.
